Please list the drug(s), dose, frequency of administration and total quantity of drug that you are requesting for this horse. Every request will need checking by a vet before this request can be authorised.

Please confirm whether you want Hampton Equine Vets Ltd to supply the drugs, or to send a written prescription directly to a pharmacy of your choice. If your request is accepted, payment will be required prior to dispensing medication, or sending a prescription.
